Includes original documents, 13 total including one US MSM certificate, five Army Commendation Medal certificates, two Army Achievement Medal certificates, two Honourable Discharge Certificates and one Oath of Re-enlistment Certificate. Certificates span from 1970 to 1992. Not all certificates are displayed
Documentation with the portfolio indicates that he received the Bronze Star with “V” device and Air Medal during his career (certificates not with group). CSM Vazquez-Ortiz served in Operation Uphold Democracy (19 September 1994 – 31 March 1995) and Operation Restore Hope (Somalia 5 December 1992 – 4 May 1993) in later years. Quite possibly he received the Bronze Star with “V” device, Purple Heart and Air Medal in Vietnam.
This is a fine collection of original US Army documentation for 097-40-8631 CSM Carlos Vazquez-Ortiz spanning 20 years military service with the US Army. His 1995 DA Form 638-1 indicates that he received the following awards up to that point Bronze Star with V (2), Purple Heart (1), Meritorious Service Medal (2 Oak Leaf Clusters), Army Commendation Medal (5 Oak Leaf Clusters), Army Achievement Medal (3 Oak Leaf Clusters), Air Medal (1)

Citation:
Specialist Five Carlos O. Vazquez, 097-40-8631, US Army, performed exceptionally outstanding service as Driver, Protocol section, HQ XVIII Airborne Corps at Fort Bragg, Fort Bragg, North Carolina, from 9 Sept 1970 to 29 Oct 71. Throughout this period, SPC Vazquez demonstrated superb efficiency in all phases of his assigned tasks. Adapting readily to changing situations and requirements, he approached each task with enthusiastic zeal, demonstrating a high degree of initiative, professionalism and leadership. He represented the headquarters in an excellent manner when he was required to drive for visiting dignitaries, both foreign and domestic. His leadership abilities were amply demonstrated when he was required to coordinate overall transportation requirements for a visiting group. His military bearing and attitude, cheerfulness and cooperative spirit resulted in numerous favourable comments from visiting dignitaries. He was on call twenty four hours a day and always performed in an exceptional manner whenever called upon to do so. His dedication to duty, thoroughness in detail and persistence in accomplishing the assigned job contributed significantly to the successful operation of the Protocol Section. The outstanding manner in which SPV Vazquez performed his duties reflects great credit upon him, his organization and the military service.
Army Commendation Medal (Second Oak Leaf Cluster) for Meritorious Service from 8 February 1975 to 29 October 1976
For Meritorious Service while serving as a light vehicle operator, Transport Motor Pool, Directorate of Industrial Operations, Headquarters, Fort Drum, Fort Drum, New York, from 8 February 1975 to 29 October 1976. During this period SGT Vazquez performed his duties in an outstanding manner. His invaluable assistance and responsiveness in operations support enabled units to conduct meaningful and realistic training at Fort Drum. He demonstrated courtesy, promptness and efficiency in ensuring the scheduling of drivers and vehicles in accomplishing the mission. In all undertakings he displayed unique competence, superb leadership and inviolate professional integrity. Sergeant Vazquez’s commendable achievements, loyalty and devotion to duty are in keeping with the proudest traditions of the US Army and reflect great credit upon himself and the military service.
US Meritorious Service Medal 27 July 1978 Newburgh District Recruiting Command
For meritorious service in support of the US Army Recruiting Command’s Qualitative Incentive Procurement System. SGT Vazquez-Ortiz has sustained a level of performance achieved only by a select group of Army recruiters. This attainment reflects an exceptional ability to successfully meet the demanding challenges of maintaining the strength of the Volunteer Army. He clearly stands in the vanguard of top recruiters in the Command.

For meritorious service while assigned to the Newburgh District Recruiting Command from December 1976 to October 1982. SGT Vazquez served in the positions of field recruiter, Harlem Recruiting Station, Station Commander, Times Square, Recruiting Station and Station Commander, Harlem Recruiting Station. He distinguished himself as a recruiter earning the top award in the US Army Recruiting Command: The Recruiter Ring. As a Station Commander, his station was the Top Producing Station in Manhattan and in the District recruiting Command. He is a true professional who leads by example and demands excellence from himself and those who work for him. SGT Vazquez’ dedication, determination and drive reflect great credit upon him, the Recruiting Command and the US Army.
Army Achievement Medal on 23 June 1983
For meritorious achievement as the field service and training NCOIC while assigned to the S2/3 section of Headquarters Detachment, 2D Supply and Transport Battalion from October 1982 to December 1982. During this brief period SFC Vasquez completely reorganized the training program of this battalion. His efforts and outstanding accomplishments were rewarded during the 2D Infantry Division Annual General Inspection when all areas passed in a highly commendable manner. His professionalism as expressed in his outstanding performance of duty, impeccable bearing and positive attitude sets a high standard for all to follow and reflects great credit upon him, the 2D Infantry Division and the US Army.
Army Achievement Medal on 28 July 1983
For exceptionally meritorious service from 6 May 1983 to 14 July 1983 as Platoon Sergeant, Company A, 1st Battalion (Mechanized) 17th Infantry, 2D Division. During the demilitarized zone mission SFC Vazquez’ capable leadership inspired all personnel who served with him to such an extent that all assigned missions were performed consistently in an exemplary manner. SFC Vazquez’ outstanding performance of duty is in keeping with the finest traditions of the military service and reflects great credit upon himself, the 2D Infantry Division and the US Army.
Army Commendation Medal for Meritorious Service on 31 October 1991
For exceptionally meritorious achievement while assigned as the Commander and First Sergeant of the Marksmanship Training Unit Number 1 from 5 June 1991 to 11 September 1991. First Sergeant Vazquez was solely responsible for the organization and execution of the disestablishment of his unit. His exemplary performance reflects great credit upon himself and is in keeping with the proudest traditions of the US Army.
